Each team member is a Certified Dementia Practitioner, and we also have an Aging in Place Specialist that can assist if remaining home is your ultimate goal.

#1 Determine your needs
Let’s discuss your unique situation over the phone and/or set up an in person meeting. We will customize our consultation to match appropriate communities, with consideration for overall medical/functional needs, desired amenities, geographical preferences, budget, and other factors.

#2 Educate, Recommend & Tour
Once we have determined communities to likely best meet your needs, we then streamline the process for you! We will coordinate and arrange tours, accompany you on the tours whenever possible, serving as your advocate, and making sure your questions are properly answered. We offer questions throughout the tour, you might not think to ask. We will also assist with necessary paperwork.
*Our team is comprised of only licensed healthcare professionals, with extensive clinical knowledge and senior living expertise, helping to holistically serve our clientele in a meaningful way.
#3 Continual Support & Follow-Up
It’s important to us that transitions into any senior living community are as smooth and stress-free as possible. After moving into a retirement setting, we keep in touch! It’s fairly common to see us visit and “check in”, as we are in and out of each community on a regular basis, ensuring quality care measures are being maintained.

Dementia Resource/ Aging in Place Consultations
Our licensed Occupational Therapist and Aging in Place specialist can assist with aging in place consultations. Initial consultations are at no cost, in depth assessments do have costs associated.
Dementia education, resources, and caregiver training can be provided to families as needed.
